Please send all requests within 30 days of the effective date!
Things to check before sending:
- Complete and accurate applications:
- Social Security Number
- Date of Hire
- Date of Birth
- PCP – If plan requires it
- Correct Eligibility
- Effective Date meets new hire rules
- Loss of Coverage section is completed in full if Applicable
- Documentation to support any Qualifying Events
- Marriage/Birth Certificate, etc.
- Plan Changes
- If a group makes a benefit change confirm that the correct application is used for the new plans
