Please send all requests within 30 days of the effective date!

Things to check before sending:

  • Complete and accurate applications:
  • Social Security Number
  • Date of Hire
  • Date of Birth
  • PCP – If plan requires it
  • Correct Eligibility
  • Effective Date meets new hire rules
  • Loss of Coverage section is completed in full if Applicable
  • Documentation to support any Qualifying Events
  • Marriage/Birth Certificate, etc.
  • Plan Changes
  • If a group makes a benefit change confirm that the correct application is used for the new plans
